In addition to filling out the online application, you must submit paper copies of four documents to complete your application.
For the first two documents, please print and complete the forms below. Include the completed forms in the packet of required documents that you bring to the Early Education Office.
For the next two documents, bring the original documents (not photocopies) to the Early Education Office. We will photocopy the original documents and return them to you.
Birth Certificate
Proof of Residency (mortgage statement or rental lease and utility bill).
Deadline for required documents
All four of the above documents must be received in the Early Education Office by Friday, March 27, 2026 at 4:00 p.m. for your application to be complete.

All four of the required documents must be brought or sent to Brenda Beck in the Early Childhood Office by Friday, March 27, 2026.
We no longer accept any of the required application documents via email.

Drop off outside of regular hours
If you need to drop off your paperwork when the office is closed, put your documents into an envelope addressed to Brenda Beck and place the envelope in the black mailbox outside the main entrance to Pioneer Valley Regional School.
Ms. Beck will confirm receipt of your documents via email and will send the original documents (birth certificate and proofs of residency) back to you.
